Heart palpitations: what is it and why can it threaten your life?

Palpitations of the heart, technically referred to in Latin as palpitatio cordis, represent an unpleasant perception of heart activity. It may be a rapid beating, an irregular rhythm, skipping heartbeats or a feeling that the heart is ‘bouncing’ or ‘beating in the chest’. Although sometimes this is a harmless symptom of stress or fatigue, in other cases it may be a signal of serious heart disease.

Untreated arrhythmias can lead to:

  • Blood clots
  • Stroke
  • Heart failure
  • Sudden cardiac arrest

Palpitations can be a sign of serious heart disease, such as atrial fibrillation, coronary artery disease or heart failure.

Many people tend to make light of unpleasant palpitations. However, without a professional examination of the heart, it is impossible to tell whether it is a normal stress response or a serious heart rhythm disorder. The doctor will use echocardiography, Holter monitoring (pressure holter) and other methods to determine the exact cause of the problem and suggest appropriate treatment.

How do you know if you're having heart palpitations?

Are you suffering from any symptoms of cardiac arrhythmia? Some of the most common include:

  • Feeling your heart pounding at rest
  • Rapid or irregular heartbeat
  • Skipping heartbeats
  • Feeling pressure or fluttering in the chest area
  • Feeling faint, dizzy or short of breath
  • Feeling anxious or panicky
  • Poor breathing

Palpitations are usually felt in the chest area, but can also radiate to the neck or along the upper limbs.

Triggers of palpitations

Palpitations can have a number of causes, some patients complain of palpitations when lying down, others during activity, some deal with heart palpitations at night, others during the day and physical exhaustion. The causes may be trivial, but others are serious. Common triggers include:

  • Long-term stress, anxiety, panic attacks
  • Excessive caffeine, alcohol or nicotine consumption
  • Lack of sleep
  • Physical exertion or dehydration
  • Hormonal changes (e.g. Pregnancy, menopause)
  • Cardiac arrhythmias or other heart conditions
  • Thyroid - hyperthyroidism
  • Certain medications or drugs

Types of palpitations (heart palpitations at rest) and when to be alert

Palpitations can be classified by origin:

  • Supraventricular arrhythmias (e.g. Atrial fibrillation (e.g. atrial fibrillation)
  • Ventricular extrasystoles (skipping of the heartbeat)
  • Sinus tachycardia (rapid regular rhythm)
  • Bradycardia (too slow a heartbeat)

If palpitations last longer, occur regularly or are accompanied by chest pain, shortness of breath, fainting or fatigue, seek medical attention immediately.

 

How is palpitations treated?

If you are troubled by heart palpitations, we do not recommend reading discussions and advice on discussion forums. They often mention generalities, but the therapy is very individual as it depends on the cause.

  • Lifestyle modification - limiting stress, caffeine, alcohol (some patients suffer from palpitations after food or palpitations after alcohol)
  • Treatment of the underlying disease
  • Ablation - a procedure to remove pathological electrical pathways in the heart
  • Pacemaker implantation - in severe cases
